Tricity’s first case of G-POEM successfully done at Alchemist Hospital

Panchkula: A rare endoscopic procedure known as Gastric Per Oral Endoscopic Myotomy (G-POEM), a endoscopic surgical procedure used to treat delayed gastric emptying was performed successfully in Alchemist hospital by Dr Zaheer, Senior Consultant Gastroenterologist from AIG Hyderabad & Dr Daksh Khurana, Consultant Gastroenterologist at Alchemist Hospital, Panchkula. This is a relatively new procedure performed using endoscopic technology and is the first ever case performed in Tricity.

Dr Daksh informed that patient Kanta (52 year old) resident of Kurukeshtra admitted in Alchemist Hospital with the complaints of severe abdominal bloating was not able to eat anything due to early satiety from many months. Gastric Per Oral Endoscopic Myotomy (G-POEM) procedure was successfully done on the patient for a condition called as refractory gastroparesis. In simple language this can be expressed as delayed emptying of stomach contents into the intestine leading to symptoms of early satiety, postmeal abdominal fullness, nausea, vomiting and bloating. In this condition there is significantly delayed gastric emptying in the absence of mechanical obstruction.

Dr Daksh further told that two cases of Per-oral Endoscopic Myotomy (POEM) were also performed; this surgical procedure is performed totally endoscopically.   It is performed in very few centers.  He added that this procedure is done for a condition called as achalasia cardia, a condition in which patient has trouble swallowing anything and vomits whatever is swallowed. The patient feels distressed at night due to recurrent food regurgitation in wind pipe.

 This procedure is endoscopic counter part of Hellers Myotomy(surgical procedure). This is the most recent treatment for achalasia cardia and is scarless, less costly and has shorter hospital stay.

Panchkula:  Life of a 76 year old man saved in Alchemist Hospital with a new endoscopic technique, for an uncommon condition known as Boerhaave’s syndrome.

76years old patient Mr. Tarsem Singh was admitted in Alchemist Hospital with the history of recurrent bouts of vomiting with brown coloured vomitus associated with severe pain in upper abdomen and lower chest region. 

The patient underwent endoscopy by Dr. Daksh Khurana Sr.Gastroenterologist Alchemist Hospital, which showed a linear tear at junction of food pipe (esophagus) & stomach & small fistulous opening at the top end. CT Scan also confirmed the above findings. In the past this

condition used to be treated by surgical intervention.

Patient Tarsem Singh was diagnosed to have a condition called as Mallory Weiss tear with Boerhaave’s syndrome in medical terminology. He is a known diabetic / hypertensive and a patient of heart  disease on blood thinning medication.  He was a very high risk candidate for surgery in view of old age and comorbidities. A new endoscopic technique was done on him, which saved the life of the patient and avoided surgery. Complete closure of the hole in food pipe was done with over the scope clip (OTSC) with endoscopy.

Patient recovered well from his disease and is on regular follow up and eating well.

Mallory-Weiss syndrome is a condition marked by a tear in the mucous membrane, or inner lining, where the esophagus (the tube that connects your throat to your stomach) meets the stomach. Severe and prolonged vomiting can result in tears in the lining of the esophagus, while Boerhaave's syndrome is spontaneous rupture of the oesophagus associated with the result forceful vomiting which is one of the most lethal diseases of the gastrointestinal tract, with a very high mortality rate.
